What is case management?

If you are reading this, it is likely that you were referred to Transcend Rehabilitation by your lawyer or insurance company. At Transcend Rehabilitation, our Case Managers assess, plan, implement, coordinate, monitor, and evaluate the services you need to meet your health, social care, education, and employment needs as well as rehabilitation goals. We follow the guidance and standards provided by the Case Management Society UK and Institute of Registered Case Managers.


Here's what you can expect from our Case Managers:

  • Assessment: We will assess your overall and immediate rehabilitation needs.

  • Rehabilitation plan: We will create a plan to address your needs and any obstacles you may encounter during your recovery. Our ultimate goal is to help you achieve meaningful goals and your rehabilitation potential.

  • Implementation: We will work with you to implement the plan at a pace that is comfortable for you.

  • Monitoring: We will monitor your progress and adjust the plan as needed based on your injury and any obstacles you face.

  • Coordination: We will coordinate all the rehabilitation support you need, including accessing public or private services, therapy, financial support, education, vocational support, hobbies, and lifestyle choices.

What will your Rehabilitation Case Manager be like?

All our case managers are qualified and highly experienced healthcare professionals; approachable, friendly, easy going, and most importantly trained to advocate for your rehabilitation needs and act in your best interests

You will be made to feel comfortable, so that you can be open with them and put your trust in them to support you whilst on your rehabilitation journey.

Our Case Managers and senior management team have a combined clinical and industry experience of over 50+ years with extensive experience in doing work within the personal injury / case management sector.

How will my Rehabilitation Case Manager treat me?
